What should I expect during my initial visit or communication with NYPT?

Please bring your insurance card, a copy of any diagnostic imaging (MRI, Xray and such) if any was performed. This will allow us to further verify your insurance benefits and provide you with an estimate of coverage. What to Bring to Your First Visit: 1)Your insurance information (we’ll make a copy of your card). 2)Completed patient forms -- please fill them either electronically or printed. 3)If your visit is related to an auto accident or workers’ compensation claim, bring your claim number along with contact information for your case manager or insurance adjuster. Please arrive 15 minutes early to allow time for any remaining paperwork and to make the most of your evaluation.
